for aging adults at every level of
care. Our goal is to keep the aging
adult at home as long as it’s safe
and practical.

We assist seniors and their
families before, during, and after
a crisis. We offer comprehensive
assessments at home, the hospital,
skilled nursing and assisted living
facilities. Our team of Geriatric
Care Managers will:

• Assess current living arrangements and care needs
• Develop a plan of care specific to the aging adults needs
• Set up services in the home to promote independence
• Provide ongoing follow up and quality of care monitoring
• Assist with transitions to alternative living situations

 

 

  Our services are designed to assist families and seniors navigate the complicated health care system, and help them make informed decisions about the type of care needed and their available resources to pay for the care.
